Service Scope
True Care Home Health of Northern California provides professional home healthcare services including but not limited to:
- Skilled nursing services by licensed RNs and LVNs
- Physical, occupational, and speech therapy
- Advanced practice nurse services
- Certified home health aide services
- Medical social work services
- Assisted living waiver programs
Physician Orders Required
All home healthcare services must be ordered by a licensed physician and are provided in accordance with an established plan of care. Services are delivered by licensed healthcare professionals under physician supervision.
Emergency Services
While we provide 24/7 emergency support, True Care does not replace emergency medical services. In case of a medical emergency, call 911 immediately before contacting our emergency line at (916) 720-2770.
Your Rights as a Patient
- To receive respectful, considerate, and compassionate care
- To be fully informed about your condition, treatment, and prognosis
- To participate in the development of your plan of care
- To have your personal and medical information kept confidential
- To voice complaints and grievances without fear of retaliation
- To choose your healthcare providers within insurance limitations
- To refuse treatment or discharge from our services
Your Responsibilities as a Patient
- Provide accurate and complete health information
- Follow your prescribed plan of care and treatment
- Inform us of changes in your condition or medication
- Treat our staff with respect and courtesy
- Provide a safe environment for care delivery
- Pay for services as agreed or provide insurance information
- Notify us of scheduling changes or cancellations

Payment and Billing
- Services are billed according to current Medicare, Medi-Cal, and insurance guidelines
- Patients are responsible for copayments, deductibles, and non-covered services
- Payment is due within 30 days of billing unless other arrangements are made
- We accept Medicare, Medi-Cal, private insurance, and private payment
Insurance Verification
We verify insurance benefits prior to service delivery, but benefit verification is not a guarantee of payment. Patients remain responsible for services provided regardless of insurance coverage decisions.
