Healthcare Services
Service Description
True Care provides professional home healthcare services including but not limited to:
- Skilled nursing care by licensed RNs and LVNs
- Advanced practice nurse services
- Physical, occupational, and speech therapy
- Certified home health aide services
- Medical social work services
- Assisted living waiver program coordination
Physician Orders
All healthcare services require a physician's order and plan of care. Services will be provided in accordance with the established plan of care and physician orders. Changes to care plans must be approved by the attending physician.
Eligibility Requirements
- Must be homebound or meet specific criteria for home health services
- Require skilled nursing or therapy services
- Have a safe home environment for service delivery
- Provide accurate medical and insurance information
Patient Responsibilities
Cooperation with Care
- Provide accurate and complete medical history
- Follow prescribed treatment plans and care instructions
- Keep scheduled appointments or provide adequate notice of cancellation
- Maintain a safe environment for healthcare providers
- Treat all staff with respect and courtesy
Financial Responsibilities
- Provide accurate insurance information
- Pay applicable co-payments and deductibles
- Notify us of any changes to insurance coverage
- Be responsible for services not covered by insurance
Communication
- Report changes in condition or concerns promptly
- Participate in care planning and goal setting
- Ask questions about your care when needed
- Provide feedback about services received
Billing and Payment
Insurance Billing
We accept Medicare, Medicaid (Medi-Cal), and most private insurance plans. We will bill your insurance directly as a courtesy. However, you are ultimately responsible for all charges related to your care.
Payment Terms
- Co-payments and deductibles are due at the time of service
- Patient responsibility amounts are due within 30 days of billing
- Payment plans may be available for qualifying patients
- Services may be suspended for non-payment
Cancellation and Discharge
Patient-Initiated Discharge
You have the right to discontinue services at any time. We request 24-hour notice when possible to ensure proper care coordination and discharge planning.
Agency-Initiated Discharge
True Care may discharge patients under the following circumstances:
- Goals of care have been met
- Patient no longer requires skilled services
- Unsafe environment for staff or patient
- Non-compliance with treatment plan
- Non-payment for services
- Abusive or threatening behavior toward staff
Limitation of Liability
True Care's liability is limited to the direct costs of services provided. We are not liable for:
- Indirect, incidental, or consequential damages
- Treatment outcomes beyond our control
- Pre-existing medical conditions
- Patient non-compliance with treatment plans
- Emergency situations requiring immediate medical attention
Professional Standards
Licensing and Certification
All healthcare professionals employed by True Care maintain current licenses and certifications required by California state law and regulatory agencies.
Quality Assurance
We maintain quality assurance programs to ensure high standards of care and compliance with all applicable healthcare regulations and accreditation standards.
Emergency Situations
Important: True Care home health services are not a substitute for emergency medical care. In case of a medical emergency, call 911 immediately.
Our services are provided during regular business hours unless specifically arranged for extended coverage. For urgent situations outside business hours, contact your physician or seek appropriate emergency medical care.
